AN ACT REQUIRING A TRANSITION PLAN FOR PERSONS WITH INTELLECTUAL DISABILITY WHO ARE A RESIDENT OF A COMMUNITY-BASED RESIDENTIAL FACILITY OR PARTICIPANT IN A DAY CARE PROGRAM, DAY CAMP PROGRAM OR RECREATIONAL PROGRAM.

To require the Department of Developmental Services to develop a transition plan for persons with intellectual disability who are a resident of a community-based residential facility or a participant in a day care program, day camp program or recreational program.
