AN ACT CONCERNING PATIENT-DESIGNATED CAREGIVERS.

Be it enacted by the Senate and House of Representatives in General Assembly convened:

That chapter 368v of the general statutes be amended to require each hospital, as permitted under federal law, to (1) provide each patient, upon the patient's admission to the hospital, with the opportunity to designate a person as a caregiver, (2) notify each designated caregiver when the patient is discharged from the hospital to another facility or to the patient's home, and (3) provide the designated caregiver with instructions on how to provide care for the patient when the patient returns to his or her home.

Statement of Purpose:

To support family caregivers and promote aging in place.
