"AN ACT CONCERNING ACCESS TO BIRTH CERTIFICATES AND PARENTAL HEALTH INFORMATION FOR ADOPTED PERSONS. "

Change the effective date of sections 1 to 11, inclusive, to "Effective January 1, 2015"

Strike section 3 in its entirety and insert the following in lieu thereof:

"Sec. 3. (NEW) (Effective January 1, 2015) (a) Not later than January 1, 2015, the Department of Public Health shall develop a health history form to be used for a birth parent to record his or her health history. The department shall provide each birth parent considering placement of a child for adoption with such health history form. The completed form shall be returned to the department.

(b) When the department receives the completed health history form from a birth parent, the department shall maintain such form in a confidential file and shall provide copies only to the adopted person who is eighteen years of age or older or such adopted person's adult child or grandchild, upon request. A completed health history form shall not be considered a public record for the purposes of section 1-210 of the general statutes. "
