The following forms may be printed on your printer. Please fill out the applicable form and deliver it to the State Capitol Police administration office in room 1200 of the LOB.

Flag Request - Use this form when making a request to fly a flag on the center flagpole.

The following forms require local access to the General Assembly's Intranet site.

ID Badge Request Form - Use this form when making a request for an ID access badge.

ID Badge Replacement Form - Use this form when making a request for an ID badge that was lost or stolen.

Room Access Request Form - Use this form when making a request to add room access to your ID badge.

Lock Change Request Form - Use this form when making a request for a key or lock change to a desk or file cabinet.
