"AN ACT IMPLEMENTING THE GOVERNOR'S BUDGET RECOMMENDATIONS CONCERNING AN ALL-PAYER CLAIMS DATABASE PROGRAM. "

After the last section, add the following and renumber sections and internal references accordingly:

"Sec. 501. (NEW) (Effective from passage) A health care provider, as defined in section 19a-127o of the general statutes, shall provide notice to a patient at the time services are provided that the patient's health care information may be included in the all-payer claims database, as described in section 1 of this act, and allow such patient to deny the use of such information for such purpose. "
